administered. Does your child have problems with the medication
that the school should know about.

should provide the school
with an Epi Pen to use if your child has an allergic reaction

Asthma
|
Request
a second inhale from
pharmacy to be maintained at school.

If
your child asthma is severe, you should request a note from doctor so
your
child can keep their inhaler with them

Request
an extra spacer for
child to keep at school also
|
Behavior
Problems
|
Type
of medication your child is taking as well as times they are

Diabetes
|
Updated
care plan from your
doctor which will include: Range for blood/sugars, guidelines for high
and low sugars, insulin doses, ketones checks, snacks
|
You
should supply the school with healthy snacks for your child for the
entire the
year. Doesn't have to bring them in all at once. A supply
of glucose tabs and a current syringe and vial of Glucagon needs to be
maintained in the nurse's office.

Seizures
|
How
often does your child have seizures and what care/notifications should
be provided afterwards. The type of medication your child is
taking and the name of your primary care provider.

Surgery
|
You
should indicate type of surgery and the date if it has been a recent
event or if the surgery will cause your child have any difficulty
participating in normal school activities, including sporting events.
|
Stomach
|
Stomachache
are a common occurrence with school age children. 90% of the time
their is not detectable reason, but if your child has a diagnosis that
the school should be aware of, please indicate that information on
child health record
|
Skin
|
Numerous
skin eruption are seen on children and many are considered contagious
until cleared by a doctor. Please provide medical documentation
to the school if your child has a chronic condition of this type.
This will help in unnecessary missed days of school.

medical history
|
Anything
that can or may hinder
your child's ability to participate in any school related activity
should be noted.
